SALEM NH EVENING CLASS SCHEDULE:                          

 

Mar 01, 2011 to Apr 19, 2011-Must be 16 by April 19, 2011.(class meets every Tue/Wed 6:30 to 8:30pm.)

 

Apr 20, 2011 to Jun 15, 2011-Must be 16 by June 15, 2011. (class meets every Tue/Wed 7:00 to 9:00pm.)No class on Apr 26 & 27 Salem school vacation week.

I agree to attend all scheduled classes.  I also know that I must be 16 by                                                 and I have provided a copy of my birth certificate in proof of this. (Fill in date of last class date from above)

·          No student will be given a green slip, indicating successful completion of driver education course, who has missed more than two classes and has not passed the course test with a grade of 70% or higher, returned textbooks in good condition, completed and turned in all assigned classroom work and who has not paid in full all tuition and fees.

·          Driving lessons are scheduled in advance.  The driving instructor will schedule driving time with each student.  Student must be able to drive Saturday or Sunday.  Students who must reschedule a driving lesson must do so prior to 6:00 AM of the date the car lesson was scheduled.  Missed car lesson appointments will result in a fee of $35.00  A missed lessons fee must be paid in full prior to the next car lesson. 

·          To the best of my knowledge my son/daughter is not under any suspension or revocation, which would prevent him/her from obtaining a license.

Please mail enrollment form and permission form to Rockingham Driving School, PO BOX 4185, Windham NH 03087, along with a check for $600.00 or choose the budget plan.  Budgeted arraignments require a  non-refundable deposit of $200.00 with weekly payments towards the remaining balance.  All students must pay in full prior to the end of the program.  Please make checks payable to Rockingham Driving School Inc.

Rockingham Driving school reserves the right to accept this application and permission form on a first come, first serve basis. Class size and availability of driving times are limited through New Hampshire regulations.  Cancellation Policy:  No Refunds except in the case of a class cancellation. In the event of a serious illness and a student must postpone for health reasons, that student who withdraws from the program will be allowed the opportunity to attend another scheduled class within 12 months from the date of last class last attended. Rockingham Driving School reserves the right to cancel or postpone the start time and class schedules at anytime.  Any scheduled class where enrollment is less than six students will be postponed and those students effected will be offered a 100% refund or a reserved spot in the next available class.   Students and legal Guardian agree to hold harmless Rockingham Driving School, Inc. in the event of course cancellation and any delay to program.                                         revised 02/11
